    Please answer the following questions about your child. In reviewing the application during the admissions process, the information given will be used to better understand your child's history and past performance. The information given is considered to be confidential information, and if accepted, this information will become part of the student's permanent record file. Additional sheets may be used if necessary.

    **If yes, please note: Diocesan policy dictates that your doctor must complete the Diocesan Medication Form mentioned in your Parent Handbook. The form must be forwarded to the school along with the original pharmacy label attached to the medication. Should your physician suggest administering an over the counter medication, a Medication Form signed by your physician and a pharmacy label must be attached to this medication as well. This form does not need to accompany the application, but must be turned in prior to the first day of school.

We/I understand that false, misleading, or omitted information provided in this application may result in a denial of admission, or dismissal in the event of admission.

      Admissions Policy

      It is our intention to accept students who can be well served by our program. The Good Shepherd Catholic School applies all policies and procedures, including admissions, equally to all students and does not discriminate on the basis of race, religion, socio- economic status, sexual orientation, or national or ethnic origin. Application review will begin when the application is complete.

      Visits and Interview

      The Good Shepherd Catholic School requires a parent and student visit. If you have not already arranged for a parent visit, please call or e-mail us to schedule an appointment.
